- Two signed, notarized powers of attorney (Power of Attorney for Certificate of Title, Power of Attorney for Vehicle Registration)
- Proof of Full Legal Name and Date of Birth
- Your Social Security Card
- Proof of Legal Presence
- Your Ohio address
