- Your Social Security number

Do I need to take a driving test?
Nope, save your real driving skills for the road. You will, however, have to pass a vision exam.

- Your Application for Registration and Certificate of Title (form H-13)
- Your last issued out-of-state title
- Your Proof of Identification
- Proof of vehicle inspection by an authorized DMV employee (report or Vehicle Identification Number (VIN) verification)

- A written request to cancel the registration (including receipt request)
Do I need to return my license plates?
Yes. Unfortunately, you won’t be able to save them for the grandkids. When you return your plates, make sure to remove their frames and fasteners; the DMV will not accept a vehicle plate with anything still attached.
